This section of the Durham Meeting website contains numerous resources useful in considering end of life issues. These materials were originally presented in two forums in October of 2006. Listed below are the various resources available here along with a brief description. Click on the titles for immediate access.

Health Care Power of Attorney / Advance Directives (PDF form)
The North Carolina Statutory form for naming a Health Care Power of Attorney. This form also provides for most mental health issues and has space for recording standard advance directives. A critical form in the modern world of health care.
Health Care Power of Attorney / Advance Directives (Word form)
The same North Carolina Statutory form as above but in Word format. This form can then be customized to provide a wholly “typed” form without the blank lines for manually writing in information thereby creating a more legible and cleaner document.
Advance Instruction for Mental Health Treatment (PDF form)
While the Health Care Power of Attorney addresses mental health care, this document is much more specific in its instructions to treatment providers and a health care agent. It is aimed at persons who have had experience with mental health treatment and who know what specific modalities and drug regimens are appropriate for them. Like the Health Care POA, this form is produced by the Secretary of State's office of North Carolina.

Memorial / Funeral and Disposition Desires (form)
This document may be used to state desires with regard to funeral arrangements and disposition or your body. If your desires are not consistent with Durham Meeting practice, they will be communicated to another church or organization per your wishes

Notification List (form)
A form useful for listing those who you would want contacted immediately in case of incapacitation or emergency. A supplementary list is for wider friends.
Personal Information (form)
A form useful for recording vital information necessary for legal purposes and other notifications upon death
Document and Property Listing (form)
A "checklist" form useful for making a record of where important documents and property can be located. A copy may be given to the person who you expect to be the executor of your will. However, for theft or security reasons, care should be taken that this listing does not fall into the wrong hands.
Helpful Organizations
A nationwide non-profit group with local branches. Very helpful in providing information regarding various service options and local pricing practices. In addition, the FCA can provide specifics regarding body or organ donation. (A lifetime membership fee in FCA is only a one-time $25.)
A nationwide non-profit group with local branches advocating for humane choices, empowerment, and dignity at the end of life. This group not only advocates and lobbies, but also provides personal support and resources to those dealing with end of life illnesses. Worthy of Friends support.
General Power of Attorney * (PDF form)
This North Carolina Statutory form provides the basis for appointing a person to act in your behalf in all the areas of your life that you may specify. This could be especially important if you own a business or have a complex personal financial situation.*
General Power of Attorney * (Word form)
This North Carolina Statutory form is also provided in Word format so that it can be customized and printed as a “typed” document eliminating fill-in lines.
Sample Simple Will * (Word form)
This sample will provides a basic format and is acceptable in North Carolina. Additional paragraphs can be added and the guardian sections removed if there are no minor children. The “Self Proving” section (last page) could be important in the timely execution of the will. This document should be completely “typed” with no “fill in” sections except for signatures and the notary certification.
If you are named as the Executor or Personal Representative for the purposes of settling an estate, this is a listing of the various duties that you will be called upon to accomplish. *
